Only a Company or Organization that currently holds a Temporary Covered Source Permit or a Temporary Noncovered Source Permit is allowed to submit a change of location request.
At least thirty (30) days prior to the change in location, or such lesser time as approved by the Department of Health.
A change of location can only be performed by a Company or Organization that currently holds a Temporary Covered Source Permit or a Temporary Noncovered Source Permit. Prior to changing locations, the Company or Organization must first receive written approval from the Clean Air Branch. If you wish to submit the Change of Location Request in hardcopy, rather than on-line, this form is included in every Temporary Covered Source Permit and Temporary Noncovered Source Permit.
